The chapel is a mausoleum of the 19th-century owners of Tuchów – the Rozwadowski and Theodorowicz families.
It is located at the youngest cemetery in Tuchów, called the "new cemetery", located on the land transferred in 1890 by Ludwika Rozwadowska. In 1904, the countess founded there a family grave chapel, which was designed by the well-known Tarnów architect Adolf Juliusz Stapf. The chapel is a neo-Gothic red brick building with decorative architectural sandstone details. It consists of a presbytery closed on three sides and a later annex with the sacristy. Inside the building there is a neo-Gothic wooden altar with a mensa-sarcophagus and a large crucifix, as well as epitaph boards mounted on the walls. The tomb crypts are located in the underground.
