The exact time of the church's erection is not known. However, during archaeological work carried out in its foundations, a foundation stone with an engraved date of 1595 was found. Today, the stone is exposed in the base of the main altar.
The church was built in the 16th century in the former village of Terlików, near Tarnów, on the initiative of Father Łukasz Godzinka, vicar of the Tarnów collegiate church. In 1597, it was consecrated by the Bishop of Kraków, Cardinal Jerzy Radziwiłł.
It is a timber-framed, shingle-framed temple with a double-pitched shingled roof. The church's interior is single-nave with a three-sided enclosed presbytery adjacent to the sacristy and vestibule. The Baroque tower from 1837 has a columnar structure, a porch in the ground floor, and a spherical cupola with an octagonal lantern.
The modest whitened interior conceals flat ceilings with blue polychromy from the early 20th century. The neo-classical pulpit and the pillar-supported music choir date from the 19th century. A Baroque crucifix from the 17th century can be seen on the rood beam, and the late Renaissance main altar from the 17th century contains a valuable painting with a rare depiction of the Holy Trinity in Pietas Domini type, painted on board at the turn of the 16th century. The 16th-century side altars feature Renaissance paintings.
In the 19th century, the church, which was threatening to collapse, was to be demolished. In 1858, it was renovated on the initiative of Father Michał Król, pastor of the Tarnów Cathedral, with the financial assistance of Prince Władysław Sanguszko, as recalled by the inscription on the rood beam. A major renovation was also carried out in the 1990s.
